Job summary

Community Horizons LLC in Wisconsin is seeking a long-term Supportive Care Aide to build consistency and rapport in a private home setting.

You will assist with ADLs, transport to day programs, administer medications, prepare meals, support skill-building, provide companionship, and monitor changes in condition. Shifts include days and overnights with flexibility; mileage reimbursement applies.

This is a direct-hire role with no agency involvement.

Qualifications

• Experience working with adults with disabilities, non-verbal individuals as well as behavioral issues a PLUS.
• A compassionate, patient, energetic approach to caregiving.
• Strong communication skills.

  Schedule/Hours

  • Monday–Thursday, 2:00 pm– 9:45 am
  • Friday at 2:00 pm through Monday at 9:45am
  • Additional daytime hours may be available-information available upon interview (2:00pm-7:00pm)

  • Both day and overnight shifts available

  Wage

  $16.50 per hour for dayshift- plus a mileage reimbursement of .35 per mile, overnight shifts offer a flat rate stipend of $50.75
