Job summary

Benchmark Human Services in South Bend, IN seeks a Supported Living Manager to oversee operations of our Supported Living homes. You will supervise staff, support ISP/PCP development, and ensure client files meet state and federal regulations.

Work includes coordinating with families, case managers, healthcare partners, managing budgets and Medicaid documentation, scheduling medical appointments, and planning community activities for clients.

Qualifications

• Bachelor's degree preferred
• Two years' experience working with individuals with mental disabilities
• Minimum 1-year supervisory experience
• Valid CPR and First Aid
• Valid driver's license and auto insurance

Responsibilities

• Supervise, train, and provide guidance to staff in accordance with regulations and policies.
• Participate in development of each client's ISP and PCP.
• Participate in direct service for each client.
• Maintain client files and documentation per standards and regulations.
• Manage client's budget and Medicaid eligibility documentation.
• Coordinate with families, Case Managers, and healthcare partners.
