Benchmark Human Services in South Bend, IN is seeking a Supported Living Manager to coordinate operation of Supported Living homes and supervise staff. The role involves developing Individual Support Plans (ISP) and Person-Centered Plans (PCP), participating in direct service, and ensuring documentation meets state and federal standards.

Qualifications

• Bachelor’s degree preferred; experience in IDD/behavioral health is a plus.
• Two years’ experience working with individuals with mental disabilities.
• Minimum of 1 year supervisory experience preferred.
• Valid CPR and First Aid certification.
• Valid driver’s license and auto insurance.
• Must complete required training (interviewing, hiring, discipline, employee relations).

Responsibilities

• Supervise, train, and guide staff per state/federal regulations and Benchmark policies.
• Contribute to ISP/PCP development for each client as part of a collaborative team.
• Participate in direct service for clients as needed.
• Maintain client files and documentation per regulations and policies.
• Assist with Medicaid eligibility, appointments, and billing documentation.
• Communicate professionally with families, Case Manager, health professionals, schools, and landlords.
• Attend and participate in meetings (ISP, Quarterlies, IEPs) and staff trainings.
• Implement training programs per each client’s ISP and behavior plans.
• Support health care needs including medication administration and appointments.
• Plan community-based, age-appropriate activities for clients.
• Respond promptly to on-call situations and maintain required documentation.
• Ensure compliance with mandatory training and certifications; report abuse or neglect as required.
