Job summary

The Catholic Charities, Diocese of Paterson invites applications for a Support Coordinator with the Department for Persons with Disabilities (DPD). This full-time role supports adults with developmental disabilities, with a 37.5 hour workweek and a daily schedule 8:30 am–4:30 pm.

You will manage discovery, ISP development, plan coordination and monitoring, communicate with families and providers, and complete written documentation as part of a collaborative team.

Qualifications

• Bachelor's degree or higher in any field and 1 year of experience working with adults 21 or older with developmental disabilities.
• The experience must be the equivalent of a year of full-time documented experience working with adults 21 or older with intellectual/developmental disabilities.
• Experience can include paid employment, volunteer experience and/or being a family caregiver of an adult with a developmental disability.

Responsibilities

• Manage support coordination services for each participant including discovery, ISP development, plan coordination and plan monitoring.
• Communicate effectively with supervisors, participants and their families, other providers, and with DDD representatives.
• Extensive written documentation required; cooperate with agency administration and/or Division and DHS staff in inspections or investigations.
