Job summary

Catholic Charities, Diocese of Paterson's Department for Persons with Disabilities (DPD) seeks a part-time Support Coordinator in New Jersey. You will manage support coordination services for participants, including discovery, ISP development, plan coordination and monitoring, while engaging with supervisors, participants and families.

This role requires a Bachelor's degree and at least one year of experience with adults who have developmental disabilities.

Qualifications

• Bachelor’s Degree or higher and 1 year of experience with adults (21+) with developmental disabilities.
• This experience can include paid, volunteer, or being a family caregiver.
• Must have NJ driver’s license, 40-60 WPM typing, computer and organizational skills and be able to operate office equipment.

Responsibilities

• Manage support coordination services for participants, including discovery, ISP development, plan coordination and monitoring.
• Communicate effectively with supervisors, participants and their families, other service providers and DHS representatives.
• Extensive written documentation and cooperation with the agency's administration and DHS staff for inspections.
