Job summary

Catholic Charities, Diocese of Paterson invites applications for a Support Coordinator to manage participant services, development of ISPs, and plan monitoring. You will communicate with families, supervisors, and providers while ensuring accurate documentation and compliance with regulations.

The role requires a Bachelor's degree or higher and at least one year of relevant experience with adults who have developmental disabilities; NJ driver's license is required and travel is expected across

Qualifications

• Bachelor's degree or higher in any field and 1 year of experience working with adults with developmental disabilities.
• The experience must be the equivalent of a year of full-time documented experience with adults (21+) with IDD.
• Experience can include paid work, volunteer work, or being a family caregiver of an adult with IDD.
• Must have a valid New Jersey driver's license; travel may be required.

Responsibilities

• Manage support coordination services for each participant, including discovery, ISP development, plan coordination and plan monitoring.
• Communicate effectively with supervisors, participants and their families, other service providers and DHS representatives.
• This job requires significant written documentation and cooperation with the agency's administration and/or DHS staff during inspections or investigations.
