Job summary

The Columbus Organization in Newark, DE is seeking a Manager of Care Management to support statewide operations, strategic planning and service coordination. You will mentor a team of Community Navigators and Team Leaders, promoting quality, compliance, and continuous improvement across the team.

The role requires a Bachelor’s in Human Services or related field, with five years of case management experience and supervisory work.

Qualifications

Responsibilities

  • Supervise Team Leaders and ensure timely completion of contract deliverables.
  • Conduct supervision, performance evaluations, and goal setting.
  • Oversee onboarding/offboarding, system access, and training compliance.
  • Lead team meetings and trainings; ensure training requirements are met.
  • Monitor caseloads, assign individuals, and manage transitions between agencies.
  • Support Director in contract compliance, service quality, and reporting accuracy.
  • Assist in developing annual work plans aligned with strategic goals.
  • Foster a collaborative, respectful, and inclusive team environment.
  • Ensure compliance with agency policies and regulatory requirements.
  • Other duties as assigned.

Summary of position

The Manager of Care Management supports the Executive State Director in overseeing statewide operations, strategic planning, and service coordination. This role ensures the effective delivery of services for individuals with intellectual and developmental disabilities by guiding a team of Community Navigators and Team Leaders. The Manager serves as a mentor, promoting quality, compliance, and continuous improvement across the team.

Essential Functions
  • Supervise a team of Team Leaders, ensuring timely completion of all contract deliverables.
  • Conduct regular supervision, performance evaluations, and goal setting for team members.
  • Oversee onboarding/offboarding processes, system access, and training compliance in collaboration with internal departments.
  • Facilitate team meetings, in-service trainings, and ensure adherence to training requirements.
  • Monitor caseload ratios, assign individuals, and manage transitions between agencies.
  • Support the Executive Director in monitoring contract compliance, service quality, and reporting accuracy.
  • Assist in developing and implementing annual work plans aligned with strategic goals.
  • Foster a collaborative, respectful, and inclusive team environment.
  • Ensure compliance with all agency policies and regulatory requirements.
  • All other duties as assigned.

Requires Bachelor’s degree in Human Services or a related field or Associate degree in Human Services plus 2 years of experience in related field. Master’s degree is a plus. A minimum of five years of case management experience, including at least two years in a supervisory capacity. Demonstrated experience working with individuals who have intellectual and developmental disabilities. Prefers one year case management supervisor experience.
