The Columbus Organization in Newark, DE is seeking a Manager of Care Management to support statewide operations, strategic planning and service coordination. You will mentor a team of Community Navigators and Team Leaders, promoting quality, compliance, and continuous improvement across the team.
The role requires a Bachelor’s in Human Services or related field, with five years of case management experience and supervisory work.
The Manager of Care Management supports the Executive State Director in overseeing statewide operations, strategic planning, and service coordination. This role ensures the effective delivery of services for individuals with intellectual and developmental disabilities by guiding a team of Community Navigators and Team Leaders. The Manager serves as a mentor, promoting quality, compliance, and continuous improvement across the team.
Requires Bachelor’s degree in Human Services or a related field or Associate degree in Human Services plus 2 years of experience in related field. Master’s degree is a plus. A minimum of five years of case management experience, including at least two years in a supervisory capacity. Demonstrated experience working with individuals who have intellectual and developmental disabilities. Prefers one year case management supervisor experience.
Ability to travel regularly and work evenings or weekends as needed.
