Job description

Join to apply for the Support Coordinator role at Healthfirst.

The Support Coordinator is responsible for assisting care/case managers with non-clinical activities such as creating cases and events, providing telephonic outreach to members, providers, and community organizations, handling member mailings, faxing clinical requests, and managing Individual Health Care Plans on behalf of care/case managers. The role is assigned to a specific clinical team (e.g., Care Management, Utilization Management, Behavioral Health) and may include additional departmental responsibilities.

Position Details:

  • This position is 100% remote.

Duties and Responsibilities:

  1. Provide excellent customer service to members and providers regarding authorization updates, care coordination, scheduling, and assessments to identify risk factors requiring intervention.
  2. Handle calls to/from providers about authorizations, referrals, visits, tests, and care plans.
  3. Manage requests from members, providers, call centers, and care teams regarding authorizations and updates.
  4. Complete notifications to members and providers.
  5. Escalate calls to appropriate departments as needed.
  6. Manage a high volume of tasks across multiple queues.
  7. Meet or exceed performance, quality, and productivity standards.
  8. Build trust through open communication with internal and external stakeholders.
  9. Accurately document member information per internal procedures.
  10. Follow policies to address member and provider issues timely and accurately.
  11. Communicate member issues effectively to improve outcomes.
  12. Maintain confidentiality of Protected Health Information (PHI) in compliance with HIPAA and other regulations.
  13. Perform additional duties as assigned.

Minimum Qualifications:

  • High School diploma or GED.
  • Experience in customer service.
  • Ability to work in a fast-paced environment.
  • Proficiency in Microsoft Office applications.

Preferred Qualifications:

  • Languages: Spanish, Russian, French, Creole, Mandarin, Cantonese.
  • Knowledge of medical terminology.
  • Experience in healthcare, managed care, or call centers.
  • Experience with CRM systems.
  • Strong problem-solving and communication skills.

Salary Range:

  • Greater NYC Area: $39,208 - $52,000.
  • Other locations: $34,091 - $49,920.
