Job summary

LakeWood Health in Baudette, MN seeks a Supervisor of Community Health to lead and shape the future of community health. You will guide the Public Health department and the RHC visiting nurse program, ensuring seamless service delivery and continuous improvement.

You will mentor a team of nurses and health staff, drive quality improvement, and develop strategies that address community health needs. A sign-on bonus is available for eligible hires.

Qualifications

  • Graduate of an accredited school of professional nursing with current licensure in Minnesota (BSN).
  • Certified in Public Health Nursing by Minnesota Board of Nursing.
  • RN Licensure in Minnesota.
  • Basic Life Support (BLS) certification.
  • Valid driver’s license and auto insurance.

Responsibilities

  • Lead and shape the future of community health in the Public Health department.
  • Oversee the RHC visiting nurse program and ensure high-quality, safe services.
  • Manage budgets, contracts, and resource planning for personnel and supplies.
  • Develop, implement, and evaluate policies aligned with LakeWood Health’s mission.
  • Ensure compliance with State Department of Health standards and quality initiatives.
  • Mentor staff and promote a collaborative, positive work environment.

Job Summary and Responsibilities

As our Supervisor of Community Health, you will lead and shape the future of community health.

Every day you will be at the forefront of public health, ensuring seamless service delivery and continuous improvement.

To be successful in this role, you will lead our Public Health department and RHC visiting nurse program.

  • Maintaining the highest standards of quality and safety in all services.
  • Guiding financial oversight for personnel and supplies.
  • Developing and implementing policies that align with our mission.
  • Ensuring compliance with State Department of Health standards.
  • Fostering a collaborative and positive work environment.
  • Overseeing risk management and quality improvement initiatives.
  • Engaging with community partners and developing contracts.
  • Mentoring staff and sharing your expertise.
  • Driving emergency preparedness planning and training.
  • Collaborating on community health needs assessments and strategic planning.
  • Managing the departmental budget.

We are offering a $10,000 Sign-On Bonus to new hires who meet the eligibility requirements.
