Job summary

LakeWood Health in Baudette, MN seeks a Supervisor of Community Health to lead public health and a visiting nurse program, ensuring high-quality services. You will mentor staff, oversee budgets, engage with community partners, and drive policy development and emergency preparedness across the department.

This full-time role offers a $10,000 sign-on bonus for eligible hires and requires MN BSN, PHN certification, MN RN licensure, BLS, and a valid driver's license.

Qualifications

  • Graduate of accredited nursing school with MN licensure (BSN).
  • Public Health Nursing certification by MN Board of Nursing.
  • RN licensure in Minnesota and current BLS certification.
  • Valid driver's license and auto insurance.

Responsibilities

  • Lead and shape the future of community health in our public health department.
  • Ensure high-quality, safe, and effective services with policy implementation.
  • Oversee budget, contracts, risk management, and quality improvement initiatives.
  • Mentor nurses, health aides, and staff; engage with community partners.
  • Drive emergency preparedness planning and community health needs assessments.
  • Supervise the visiting nurse program and ensure compliance with standards.

Job Summary And Responsibilities

As our Supervisor of Community Health, you will lead and shape the future of community health.

Every day you will be at the forefront of public health, ensuring seamless service delivery and continuous improvement.

To be successful in this role, you will lead our Public Health department and RHC visiting nurse program.

  • Maintaining the highest standards of quality and safety in all services.
  • Guiding financial oversight for personnel and supplies.
  • Developing and implementing policies that align with our mission.
  • Ensuring compliance with State Department of Health standards.
  • Fostering a collaborative and positive work environment.
  • Overseeing risk management and quality improvement initiatives.
  • Engaging with community partners and developing contracts.
  • Mentoring staff and sharing your expertise.
  • Driving emergency preparedness planning and training.
  • Collaborating on community health needs assessments and strategic planning.
  • Managing the departmental budget.

We are offering a $10,000 Sign-On Bonus to new hires who meet the eligibility requirements.
