Qualifications

  • Case Management Certification within two years for Federal Employee Program incumbents.

Responsibilities

  • Supervise, coordinate, and oversee daily work of case management staff.
  • Facilitate department operations including customer requests, projects, and staffing.
  • Represent case management at internal and external meetings.
  • Lead a team of Dietitians and Respiratory Therapists.
  • Direct and supervise CM staff, including development plans and performance reviews.
  • Coordinate staff communications and daily activities and ensure adequate staffing.
  • Participate in recruitment, training, and staff retention.
  • Conduct case reviews, performance reviews, and staffing pattern reviews.
  • Monitor productivity, inter-rater reliability, and performance metrics.
  • Maintain an individual caseload and handle member clinical management programs when applicable.
  • Coordinate regular team meetings.
  • Maintain employee files and required education and annual statements.
  • Assist with CM policies, procedures, departmental changes, and initiatives.
  • Develop and implement new initiatives using project management skills.
  • Provide presentations related to supervised functions.
  • Liaise with departments, members, providers, and community agencies.
  • Identify strategies to improve health care resource management.
  • Instruct and support staff in special situations or problems.
  • Maintain departmental documentation and prepare reports including Quality Improvement reports.
  • Ensure compliance with regulatory and accreditation standards and monitor implementation of requirements.
  • Maintain professional education and knowledge of legislative and regulatory mandates.
  • Conduct staff meetings and distribute departmental and Ethics/Compliance information.
  • Perform other duties assigned by management
