Job summary

Interfaith Community Services INC in Carlsbad, CA is hiring an Outreach SUD Counselor to provide mobile outreach, screenings, and care coordination for individuals experiencing homelessness with opioid use and co-occurring disorders. The role partners with SUD outreach teams and community partners to identify needs and connect clients to services.

You will work in a multidisciplinary setting, perform field assessments, develop individualized plans, and support engagement with recovery and crisis

Qualifications

• High School Diploma or equivalent, required.
• Experience with individuals dealing with alcohol, drugs, and mental illness or homelessness, required.
• Must be registered by CCAPP/CADTP/CAADE and maintain certification, required.
• Experience in addiction treatment, social services, or nonprofit setting, required.
• Bilingual English/Spanish preferred.

Responsibilities

• Conduct outreach and other engagement activities to identify eligible individuals and households, including shelter and affordable housing properties.
• Work as part of a multidisciplinary team to coordinate client care.
• Attend and participate in outreach and care conferencing meetings.
• Collaborate with regional ED social workers, SUD counselors, and mental health practitioners for continuum of care.
• Provide harm-reduction based outreach and build rapport while identifying risks and needs.
• Perform in-the-field screening and provide assessments and ongoing reassessments.
• Develop personalized, strengths-based plans to reduce harms from drugs and alcohol.
• Arrange linkage to recovery services and support.
• Coordinate referrals to MAT, detox, and other treatment options.
