Job summary

BestCare Treatment Services, Inc. seeks a SUD Counselor to provide individual and group counseling, client assessments, and treatment planning within a patient-centered harm-reduction framework.

Responsibilities include crisis intervention, documentation in EHR, collaboration with community partners, and discharge planning to ensure seamless continuity of care for diverse populations.

Qualifications

• High School Diploma (or equivalent).
• For individuals in recovery from substance-use disorder, must have maintained continuous recovery for the previous two years at the time of hire.
• CADC-I required for SUD Counselor I (CS4).
• CADC-II required for SUD Counselor II + 2 yrs’ minimum experience as SUD counselor (CS5).
• Must maintain a valid Oregon Driver License or ability to obtain one upon hire, and be insurable under the organization’s auto liability coverage policy.

Responsibilities

• Works closely with the Treatment Team with assigned caseload of clients enrolled in the Program.
• Completes intakes, assessments and develops treatment plans using ASAM criteria to assess LOC and integration with Mental Health and SUD programs.
• Collaborates with clients on completion of individualized treatment plans with measurable goals.
• Provides individual, group, and family counseling as well as case management services.
• Conducts counseling sessions for individuals, groups, and collaborative care with treatment team and other agencies.
• Maintains documentation of client services in the EHR in a timely manner per policies.
• Discharges planning and coordination with referrals; maintains HIPAA compliance.
