Job summary

Akua Behavioral Health in California seeks a Case Manager to join a Substance Use Prevention Treatment program. You will conduct ASAM assessments, manage daily billing in the Smart Care EHR, and support client-centered care across the treatment team.

Ideal candidates have experience in SUD treatment and ASAM criteria, with strong collaboration skills to coordinate services, discharge planning, and crisis response in a busy behavioral health setting.

Qualifications

• CADC or similar certification required.
• 1+ year(s) in substance use or behavioral healthcare.
• Valid CA Driver's License, health screening and background checks may be required.

Responsibilities

• Assessment: Conduct ASAM assessments of client needs, strengths, and challenges.
• Planning: Develop personalized treatment plans using motivational interviewing techniques.
• Group Facilitation: Lead psychoeducational groups on recovery, relapse prevention, and independent living skills.
• Case management: Connect clients to mental health, counseling, medical care, housing, and vocational resources.
• Monitoring: Maintain regular contact, offer support, and intervene during crises as needed.
• Collaboration and discharge planning: Work with the treatment team and partners to ensure coordinated care and clear discharge plans.
• Documentation: Maintain timely documentation in the Drug Medi-Cal guidelines compliant system.
