Job summary

Fremont School District 79 is seeking a Substitute School Nurse to coordinate the school health program, deliver nursing services, and maintain health records for students and staff. The role requires compliance with district/state policies and Illinois State Law governing nurse practice.

The substitute nurse will provide direct nursing care, maintain documentation, and collaborate with educators and families to promote health and wellness within the school community.

Qualifications

  • Valid RN or LPN certification is required.
  • Experience in school health programs is a plus.

Responsibilities

  • Provides and/or delegates direct professional nursing services, first aid, illness, and emergency care to students and staff including nursing assessment, identifying health problems, requesting diagnosis and treatment, recommending educational modifications, providing follow-up and evaluation, and maintaining appropriate documentation.
  • Keeps a daily log of all students visiting the health office for injury, illness, medication or nurse consultation. Documents student and staff injury on appropriate forms and forwards copies to relevant personnel.
  • Provides health information and counseling for students, staff, and parents regarding their child.
  • Develops and maintains a medication protocol to safely store, administer, document, and monitor the effectiveness of medication given at school.
  • Supervises and/or provides screening and follow up for deficits in vision and hearing.
  • Maintains a user-friendly and organized health office conducive to confidential communication and services.
  • Maintains accurate emergency contact information on all students.
  • Maintains accurate medical records to assure compliance with state mandates including immunizations, physical examinations, and medical conditions.
  • Prepares and submits reports as required by the Illinois Department of Public Health.
  • Makes assessments and referrals for suspected abuse/neglect as a mandated reporter.
  • Reports communicable diseases to the Lake County Health Department when indicated.
  • Plans, implements and teaches containment procedures in the control of communicable diseases and advises on exclusion and re-admission of students with infectious diseases.
  • Consults with parents, school personnel, physicians, dentists, and/or other agencies as needed.
  • Provides training to staff regarding emergency procedures for students with severe allergies, diabetes, seizures, and other emergencies as needs arise.
  • Provides health information regarding specific students to teachers and staff as indicated to ensure student safety.
  • Participates as a member of the multidisciplinary team in the identification and evaluation of students in special education programs and writes the health component of the IEP as indicated.
  • Recommends modifications of the school program for students needing accommodations due to health deficits.
  • Develops and maintains current health care plans for students who need special nursing interventions during the school day.
  • Performs other health-related tasks as assigned by the Building Principal.

POSITION SUMMARY:

The Nurse coordinates a school health program including the delivery of services to students and staff members in order to enhance health and wellness in the school community. Duties are to be performed in accordance with standards of professional school nurse practice, district/state board of education policies and procedures and Illinois State Law regarding nurse practice.

SALARY & BENEFIT INFORMATION:
