You will have the opportunity to look beyond the diagnosis and understand the whole person, their strengths, their goals, and what matters to them. You will build trusting therapeutic relationships and use trauma-informed, culturally responsive, person-centered, recovery-oriented, and evidence-based or evidence-informed approaches to help patients engage in treatment, overcome barriers, reduce harm, and achieve their individual recovery goals.Job Functions:Conduct and/or assist with comprehensive SUD assessments that identify patient needs, strengths, risks, goals, and barriers to care.Develop, implement, review, and update individualized treatment plans in collaboration with patients and the interdisciplinary care team.Provide individual, group, family, educational, and recovery-focused counseling within your scope of practice.Use motivational interviewing and other appropriate therapeutic approaches to encourage engagement and meaningful behavior change.Help patients identify strengths, triggers, coping strategies, support systems, and resources that promote sustained recovery.Provide education related to substance use, recovery, treatment options, harm reduction, overdose prevention, wellness, and community resources.Support patients receiving MOUD while also providing comprehensive services to patients who are not receiving medication-based treatment.Work closely with medical, nursing, behavioral health, peer support, care coordination, and other team members to promote integrated care.Coordinate referrals and connections to primary care, mental health services, housing, transportation, employment, recovery programs, social services, and other community resources.Participate in treatment team discussions, case consultation, discharge planning, and transitions between levels of care.Recognize changes in patient condition, behavioral health concerns, withdrawal or intoxication concerns, safety risks, and other situations requiring additional support.Participate in crisis intervention and de-escalation consistent with training, policy, and scope of practice.Support harm-reduction and overdose-prevention efforts, including education and access to appropriate resources.Maintain accurate, timely, confidential clinical documentation and treatment records.Participate in quality improvement, compliance, chart review, team meetings, continuing education, and professional development.Contribute to an environment that promotes dignity, autonomy, cultural humility, respect, safety, recovery, and hope. Most importantly, you will be part of a team that believes people are more than their substance use disorder.
