Job summary

North Country Healthcare is seeking a Substance Use Disorder Counselor/Clinician to provide evidence-based assessment, counseling, crisis response, and care coordination within a New Hampshire Doorway program and integrated healthcare setting. You will work with adolescents, adults, and families across various care settings to develop individualized plans and improve access to treatment.

Qualifications

• Associates in Substance Abuse Counseling or related field; Masters preferred.
• Licensure as Licensed Alcohol and Drug Counselor (LADC) in NH or eligible.
• Experience with substance use disorders and integrated behavioral health.
• Strong documentation and adherence to confidentiality and regulations.

Responsibilities

• Provide evidence-based SUD assessment, counseling, crisis response, and care coordination for adolescents, adults, and families.
• Conduct bio-psychosocial assessments and risk screenings, and recommend appropriate treatment.
• Deliver individual, family, and group counseling using trauma-informed, person-centered approaches.
• Support Doorway and SBIRT services across primary care, ED, outpatient, and community workflows.
• Coordinate referrals and follow-up across continuum of care; address barriers to engagement.
• Collaborate with interdisciplinary teams and community partners to support integrated treatment planning.
• Educate patients, families, staff, and partners about SUD, overdose prevention, and recovery supports.
• Document screenings, assessments, interventions, and outcomes in the EHR timely and accurately.
