Job summary

BayMark Health Services seeks a compassionate Substance Use Counselor to guide patients through medically assisted treatments and coping strategies in an outpatient setting in Chandler, AZ.

The role includes initial assessments, individualized treatment plans, and collaboration with medical staff to support recovery and aftercare planning for chemically dependent individuals.

Qualifications

• Master’s Degree in Behavioral Science Field with 1 year of experience.
• LASAC (Associate Substance Abuse Counselor in AZ) or higher.
• Previous experience working with chemically dependent individuals is a plus.
• Must demonstrate a positive, non-judgmental, and accepting attitude toward our patient population.
• Satisfactory criminal background check and drug screen

Responsibilities

• Provide individual counseling services; direct service hours at regularly scheduled intervals; coordinate all services throughout treatment.
• Conducts initial assessment and evaluation interviews for service eligibility determination.
• Completes Bio-psychosocial assessment.
• Facilitates, establishes and reviews with each patient the initial treatment plan.
• Involve families and significant others in patient’s recovery process whenever possible.
• Respond to patient grievances and complaints.
• Develop aftercare plans and discharge plans.
• Other related duties as determined by supervisor.

Position Overview

Substance abuse counselor will be in charge with assisting patients through medically assisting treatments and offering techniques for handling opioid addiction. Counselor conducts individual sessions to assist with crisis management and coping strategies. Counselor will evaluate patients’ progress during counseling and collaborate with doctors, nurses and other counselors to assist the patients’ achieving overall outcome.

• Provide individual counseling services; direct service hours at regularly scheduled intervals; coordinate all services throughout treatment.
• Conducts individual initial assessment and evaluation interviews for service eligibility determination.
