Primary Job Function

The licensed counselor will be responsible for offering support and assistance to patients enrolled in Medication Assisted Treatment for opioid use disorder. Utilizing evidence‑based practices and a trauma‑informed approach, the counselor will help patients develop individualized coping techniques and symptom management skills that enable them to experience improved quality of life. Through collaboration with doctors, nurses and a multidisciplinary team, the counselor will play a vital role in evaluating patient progress and developing an individualized plan of care.

Essential Job Functions

• Provide individual and group counseling services; direct service at regularly scheduled intervals based on patient need, best practices and state regulations; coordinate all services throughout treatment.
• Conduct initial assessment and evaluation to determine eligibility for services.
• Complete bio‑psychosocial assessment.
• Establish individualized treatment plan with each patient and review as clinically indicated.
• Respond, as needed, to patient grievances and complaints.
• Develop aftercare and discharge plans.
• Other related duties as determined by supervisor.

Qualifications

• Must be a licensed LISW, LPCC, LPC, AADC, or ADC.
• Satisfactory drug screen and criminal background check.
