Job summary

Applegate Recovery is seeking a Full Time Licensed Counselor to provide counseling within a Medication Assisted Treatment program for adults with opioid use disorder. The role involves collaborating with physicians, nurses, and a multidisciplinary team to support patients and develop individualized care plans.

The position requires LISW/LPCC/LPC/AADC/ADC licensure, with a focus on trauma-informed care, comprehensive assessments, and aftercare planning.

Qualifications

• Initial assessment and evaluation to determine eligibility for services.
• Complete Bio-psychosocial assessment.
• Establish individualized treatment plan with each patient and review as clinically indicated.
• Respond to patient grievances and complaints.
• Develop aftercare and discharge plans.

• Provide individual and group counseling services; direct service at regularly scheduled intervals based on patient need, best practices and state regulations; coordinate all services throughout treatment.
