As the Substance Use Disorder Counselor (SUDCounselor), you will provideintegrated behavioral health services to individuals with Serious Mental Illness (SMI) and co-occurring addiction andSubstance Abuse Disorder (SUD). You will get tocollaboratewithin a multidisciplinary team to deliver client-centered, coordinated care, including case management, care coordination, recovery support, and navigation services. To be successful in this role, you will bring acommunity-based approach, providing telehealth and in-person services to meet clients where they are – in the community, office, and/or home.
Client Services & Direct Care: Provide substance use counseling, assessments, case management, recovery planning, crisis support, harm reduction education, and resource navigation for individuals and families.
Care Coordination & Collaboration: Coordinate care with treatment teams, providers, and community partners to connect clients with recovery, behavioral health, housing, medical, and support services.
Compliance, Ethics & Documentation: Maintain timely documentation and ensure compliance with agency policies, HIPAA, ethical standards, credentialing, and regulatory requirements.
Additional Support: Provide bilingual support and translation services when applicable, assist with client transportation as needed, and perform other duties to support program operations.
