Qualifications

• Excellent communication skills across organization levels and community.

Responsibilities

• Manage client linkage to substance use disorder services and social supports.
• Conduct brief counseling with clients monthly and as needed.
• Lead check-in meetings with client support members.
• Prepare and submit program reports as required.
• Provide support to help clients understand substance use and options.
• Maintain current knowledge of treatment options, funding, housing, employment programs.
• Attend training and regular supervision.
• Perform other duties as assigned.
• Maintain an ongoing log of communication.
