Reliant Medical Group, part of Optum, seeks a Per Diem Nurse Practitioner to join our Behavioral Health team as an Integrated Adult Psychiatry Provider in a hybrid role, combining telemedicine and on-site care.

The role partners with primary care and behavioral health teams to support recovery using evidence-based medication-assisted treatment, with opportunities to incorporate digital therapeutics and innovative care models.

Qualifications

• Current Commonwealth of Massachusetts APRN license.
• Board Certified as a Nurse Practitioner.
• Active Federal DEA, and Massachusetts MCSR Certificate required or in process.
• Minimum of 1-3 years of postgraduate experience treating patients with substance use disorders and dual diagnosis.

Responsabilités

• Performs physical assessments, evaluations and treatments for Reliant patients seeking treatment for substance use disorders (SUD).
• Takes a detailed history of SUD and related medical history; performs appropriate physical exam.
• Orders labs as appropriate and assesses severity of SUD to determine evaluation needs.
• Establishes and implements evidence-based plan of care related to SUD, anxiety, depression, insomnia; may include medication management and referrals.
• Communicates effectively with behavioral health and primary care teams and maintains precise documentation.
• Contributes to program development, staff training and education for BH SUD services.
• Provide clinical leadership and support administrative requirements as assigned.
• Hybrid position with telemedicine and on-site services; secure telemedicine support.
• Supervise Substance Use team staff as appropriate.
• Respond to consultation/eConsult requests from primary care.
• Participate in on-call coverage and off-hours patient care.
