Community Medical Services (CMS) is hiring a Substance Abuse Counselor to assist patients recovering from addictions to opioids and other illicit substances. Qualifications

• High school diploma or GED required; higher degrees preferred.
• QAP certification must be completed by completion of training.
• Active CDC I license or higher required.
• Behavioral health experience preferred.

Responsibilities

• Maintain and manage a caseload in compliance with regulatory standards and ratios.
• Conduct individual sessions, facilitate group counseling and didactic presentations.
• Complete comprehensive behavioral health assessments and individualized treatment plans.
• Evaluate treatment effectiveness and adjust the service plan as needed.
• Coordinate care with other recovery and community-based service providers and complete referrals.
• Include family and natural supports in the client’s treatment when appropriate.
• Maintain thorough and comprehensive client records.
• Serve as primary contact for coordination with other systems requiring clinical input.
• Assist in screening and coordinating primary care services for clients.
