Job summary

Montefiore Medical Center in Bronx, NY seeks a Substance Abuse Specialist to provide individual and group counseling within the SATP and coordinate biopsychosocial assessments, treatment planning and case management for patients in the Chemical Dependency Program.

The role requires a Bachelor’s degree and a CASAC license or equivalent, plus at least 1 year of addictions counseling experience. Hours are Mon-Fri, 8:30am–4:30pm; this is a grant-funded position within the SATP Administration.

Qualifications

• Bachelor’s degree required.
• CASAC or license in social work, mental health counseling or a related field is also required.
• Minimum 1 year work experience as an addictions counselor (preferably with methadone clients) required.
• Knowledge and ability to integrate historical and current patient issues.
• Knowledge of substance abuse counseling and treatment.
• Effectively work with adults and older adults with addiction.

Responsibilities

• Screen patients to ensure appropriateness for care.
• Complete biopsychosocial evaluation utilizing all available data, including interviews with patients and families and conferring with interdisciplinary associates.
• Develop, implement, and validate treatment plans/discharges in a timely manner in collaboration with patient, family and interdisciplinary team.
• Provide chemical dependency counseling to individuals, groups, couples and families.
• Document client progress as delineated in program policies and procedures and regulatory bodies.
• Participate in staff conferences and other meetings.
• Participate in relevant performance improvement activities, including trainings and conferences.
• Provide community outreach and education.
