Job summary

Derby Public Schools is seeking a LEAP Home Visitor to support Derby High School by conducting scheduled, voluntary home visits and building positive relationships with students and families. The role focuses on identifying barriers to attendance and connecting families with school and community resources.

The position emphasizes collaboration with the Attendance Team, ongoing follow-up, and adherence to district policies, safety procedures, and confidentiality.

Responsibilities

• Conduct scheduled, voluntary home visits to promote family engagement.
• Build positive relationships with students and families through strengths-based conversations.
• Identify barriers to attendance (academic, social, emotional, family, logistical).
• Collaborate with families to develop strategies to improve attendance.
• Connect families with school-based supports and community resources.
• Communicate with attendance and administrative teams regarding needs and interventions.
• Complete required documentation for home visits using logs.
• Participate in follow-up outreach to monitor progress and engagement.
• Maintain confidentiality and safety procedures.
• Participate in team meetings and review attendance data.
• Coordinate with school personnel and community partners for comprehensive support.
• Perform other duties to support LEAP goals.
