Job summary

Northwell Health seeks a qualified clinician to perform admissions assessments, plan patient care, and coordinate rounds with physicians and nurses. The role emphasizes high-quality patient care, documentation, and discharge planning.

Two tracks are described: Physician Assistant with NYS PA license and NCCPA certification, or Nurse Practitioner with NYS NP and RN licenses, plus a collaborative practice arrangement. BLS is required for both paths.

Qualifications

• Bachelor’s Degree from an accredited program and ARC-PA accreditation.
• Current New York PA license and NCCPA certification are required.
• BLS certification is required.
• Alternative NP track requires NP master's degree and NY NP license.
• Collaborative practice agreement or attestation is required.

Responsibilities

• Takes initial patient histories and performs physical examinations.
• Initiates, documents, and communicates the plan of care and follows up for screening and preparation.
• Performs daily patient rounds.
• Writes orders for medications, labs, and diagnostic tests.
• Interprets laboratory and test results.
• Confers with physicians, residents, nursing staff and other care providers for quality care.
• Notes discharge summaries, prescriptions, and referrals on patient charts.
• Informs patients of necessary post-discharge care instructions.
• Performs related duties; essential functions noted under applicable laws.
