Job Summary

We are looking for a Structureal Heart Clinic Nurse Coordinator to support the Cardiology department. This is a Full-Time Days position supporting Baptist Pavilion.

The main responsibilities of the Structural Heart Clinic Nurse Coordinator will be to:

• Coordinate the patient’s care to increase the efficiency and quality of care that is delivered to the patient.
• Accomplish major responsibilities of the essential job duties and to extend the responsibilities beyond the episode of care to encompass care coordination with hospital collaboration.
• Be the patient advocate and care for them in a highly supportive and personal manner and also will serve as a liaison to ensure that patients and families receive resources and services as needed.
• Be a link between inpatient and outpatient care.
