Job Summary

We are looking for a Specialty Clinic Coordinator to support the Cardiology department. This is a Full-Time Days position.

The main responsibilities of the Clinical Nurse Coordinator – Cardiology will be:

1. Coordinate patient care to increase the efficiency and quality of care delivered.
2. Manage responsibilities beyond the episode of care, including care coordination with hospital collaboration.
3. Act as a patient advocate, providing support and serving as a liaison to ensure patients and families receive necessary resources and services.
4. Serve as a link between inpatient and outpatient care.
