Job summary

All Seniors Foundation is seeking a Stroke Recovery Specialist (Neuro Rehab Nurse) to lead post-stroke rehabilitation for LA seniors at home, coordinating with PT, OT, SLP, and primary care to optimize recovery and prevent recurrence.

You will educate patients and families on stroke prevention, monitor neurological status and medications, and engage in multidisciplinary planning across clinic visits, telehealth, and home visits to support long-term wellbeing.

Qualifications

• Current CA RN license; BSN preferred.
• 2+ years stroke, neuro, or rehab nursing experience.
• CRRN or SCRN certification preferred.
• ACLS and NIHSS certification

Responsibilities

• Conduct comprehensive stroke recovery assessments using NIH Stroke Scale and functional measures.
• Develop individualized rehabilitation plans coordinating with PT, OT, SLP, and primary care.
• Educate patients and families on secondary stroke prevention (BP, A-fib, diet, exercise).
• Monitor neurological status, vital signs, and medication adherence in the home setting.
• Coordinate with neurology, cardiology, and pharmacy on integrated care plans.
• Document encounters in EHR and contribute to stroke quality metrics.

Role Overview

As Stroke Recovery Specialist (Neuro Rehab Nurse), you will lead post-stroke rehabilitation programs for seniors at home, coordinating with PT, OT, SLP, and primary care to optimize recovery and prevent secondary stroke. You will educate patients and caregivers on stroke-prevention lifestyle changes.

Key Responsibilities

• Conduct comprehensive stroke recovery assessments using NIH Stroke Scale and functional measures
• Develop individualized rehabilitation plans coordinating with PT, OT, SLP, and primary care
• Educate patients and families on secondary stroke prevention (BP, A-fib, diet, exercise)
