Job summary

Position Summary

The Stroke & Sepsis Coordinator maintains all programmatic responsibilities for the development, implementation, and maintenance of processes to evidence-based care delivery to the adult stroke and sepsis patient populations.

Minimum Requirements
  • Education: Associate degree in nursing required.
  • Experience: Three years Critical Care or Emergency Department experience. Minimum of two years charge nurse experience preferred. Experience with process improvement modalities and teams preferred. Experience in the implementation and administration of clinical quality databases preferred. Experience in healthcare quality or performance improvement preferred.
  • Knowledge, Skills, Abilities: Position requires someone comfortable with patient contact and performing as an advocate. Teaching is expected to be a significant element of the position: families, staff and local associations. Proven analytical skills with the ability to organize, maintain, process, and analyze large amounts of information and complex issues. Ability to communicate in a clear, concise and appropriate manner (verbal, nonverbal, written and graphically) with a variety of audiences. Ability to manage multiple complex assignments and projects. Exceptional customer service and effective interaction with executives, physicians, staff and patients. Strong diplomacy and collaboration skills. Sound decision-making skills. Spreadsheet and database software knowledge required.
  • License/Certification/Registration: Licensed as a Registered Nurse in the State of Pennsylvania required.

Receives minimal supervision from the Supervisor, Quality & Patient Safety.

Responsibilities
  • Essential Functions:
    • Assures all patient care standards and clinical practice guidelines are met, as defined by CMS and TJC.
    • Facilitates TJC Advance Disease Certification, Primary Stroke Center survey process.
    • Collaborates with the Accreditation Coordinator to ensure TJC reporting requirements are met, including but not limited to: Evidence of Standard Compliance data; Requirement for Improvement planning and data entry; Ongoing performance improvement measure data.
    • Assumes responsibility for the ongoing management of processes related to stroke and sepsis:
      • Coordination and facilitation of the Stroke Operations and Sepsis Steering Committees.
      • Development and maintenance of clinical practice guidelines and protocols.
      • Data entry and maintenance of disease-specific registries and databases, including but not limited to: American Heart Association-Get with the Guidelines, Stroke.
      • Coordination of Tele-Stroke partnership with Hershey Medical Center, including tele-stroke carts and data sharing.
      • Liaison between Mount Nittany Medical Center and community facilities/organizations.
      • Coordination of community education/engagement/events.
      • Coordination and presentation of educational information delivered to Medical Staff and front-line staff through various mechanisms, including but not limited to: educational activities/offerings, formal and informal presentations, posters, and bulletin boards.
    • Facilitates, supports, and co-leads the Stroke Operations and Sepsis Steering Committees, in collaboration with the Medical Director, Stroke and Medical Director, Quality.
    • Measures, analyzes, generates and presents stroke and sepsis data to identify and prioritize opportunities for improvement, using best-practice benchmarking and comparatives.
      • Performs concurrent and retrospective reviews to assess adherence to stroke and sepsis clinical quality measures.
      • Develops, implements, and maintains stroke and sepsis dashboards.
    • Coordinates and facilitates performance improvement activities.
      • Implements performance improvement teams.
      • Observes clinical processes related to stroke and sepsis care.
      • Develops, implements, and maintains concurrent and retrospective feedback mechanisms regarding stroke and sepsis performance, including but not limited to: individual provider, medical group, service-line level, and administration.
    • Participates in role‑specific professional development:
      • Stroke Coordinators of PA (SCOPA)
      • Hospital Association of PA (HAP) Sepsis offerings
  • Non-essential functions: Performs related and miscellaneous duties as assigned.
