Job summary

Bellefaire JCB in Cleveland, OH seeks an SOP Outreach Coordinator for the Street Outreach Program. The role provides supervision to interns and volunteers, coordinates community outreach, conducts client assessments, and ensures program goals align with federal grant requirements.

You will supervise staff, manage data and documentation, and collaborate with community partners to support homeless youth, aiming for Positive Youth Development and adherence to program guidelines.

Qualifications

Responsibilities

POSITION SUMMARY:

The Street Outreach Program (SOP) Outreach Coordinator is responsible for implementation and ongoing operations. He/She provides direction, coordination, plus leadership and supervision to interns. Additionally, the SOP Outreach Coordinator monitors the program’s budget and research, as well as works to promote development of the program’s community outreach and assessment of homeless youth. He/She will perform the position while working in the community. This position requires varying shifts, including evenings and weekends. Outreach is required as the position is in the community, on the street. The SOP Outreach Coordinator will ensure that staff is firmly committed to utilizing the Positive Youth Development approach. He/She will assist the Department Leadership with ensuring that the Federal Youth Services Bureau (FYSB) grant objectives and goals are carried out, while ensuring compliance with all FYSB rules and guidelines.

RESPONSIBILITIES INCLUDE:
  1. Provide supervision, coordination, leadership to program interns and volunteers involved in outreach activities.
  2. Coordinate and perform community outreach and client assessment, such as targeted missing youth canvasing.
  3. Coordinate and provide case management/service coordination as well as the ongoing aftercare services via referral and linkage.
  4. Coordinate and conduct data collection and case documentation.
  5. Represent the Agency in the development or maintenance of relationship with other organizations including community agencies, public agencies and referral sources.
  6. Maintain high standards of ethical and professional conduct.
  7. Possess and employ practical understanding of:
    1. basic child, adolescent and family development
    2. issues that are relevant to homeless youth
    3. human trafficking
    4. interventions used in helping runaway, missing, and homeless teens.
  8. Contribute to the development and maintenance of the record through the timely completion of assigned documentation in accordance with applicable licensing and accreditation regulations and standards
  1. Carry out all provisions of the FYSB grant objectives, goals and outcomes, including established rules and guidelines.
  2. Provide training to external stakeholders/community partners including but not limited to law enforcement, hospitals, DCFS, schools, non-profit agencies, faith based groups, youth, and guardians as specified in program plan.
  3. Represent the Agency in the development or maintenance of relationship with other organizations including sister agencies, public agencies, managed care companies, insurance companies, funds and referral sources.
  4. Assure that program staff participates in Agency research projects to evaluate the program’s effectiveness.
  5. Assist in the development of proposal writing and fundraising activities.
  6. Ability to qualify as an approved Agency driver.
  7. Own a cell phone to be used for on-call responsibilities.
  8. Notify Manager of Runaway, Homeless Youth with an emergency situations (i.e. in need of clinical support, AWOL, safety concern for yourself or the well being of a client or family member, a client threatening to harm themselves or others, etc.).
OTHER DUTIES:
  1. Attend scheduled staff meetings, supervision and ongoing training that will provide the skills necessary to implement the program.
  2. All required trainings, certifications and licensure must be kept current in accordance with applicable licensing and accreditation regulations and standards.
  3. Maintain high standards of ethical and professional conduct and adhere to Agency policies and procedures.
  4. Assure that program staff maintains high standards of ethical and professional conduct.
  5. Respect the privacy of clients and hold in confidence all information obtained during the client’s treatment. All client-related documents should be handled in accordance with Agency guidelines on confidential material.
  6. Other duties as assigned by management.
