Breaking Ground's Street to Home program engages with more than 2,000 street homeless individuals each year and connects 500+ with housing, medical and mental health services, and other supports. The Outreach Coordinator oversees field teams and ensures staff are deployed across shifts to meet demand.

The role applies Harm Reduction and Housing First approaches, triages urgent requests, and prioritizes developing situations from the field to guide placement options and services.
