Job summary

Healthcare in Action is seeking a Peer Navigator for Hollywood to support individuals experiencing homelessness through peer-based education, housing and resource navigation, and care coordination. This patient-facing role operates in outdoor settings and requires reliable transportation and a valid driver's license.

The position offers a base salary and a 401(k) with employer match, plus generous PTO and holidays.

Qualifications

• G.E.D./High School, required plus 3+ years of work experience.
• Associate's degree preferred plus 2+ years of work experience defined below.
• Bachelor’s degree preferred plus 1 year of work experience defined below.

Responsibilities

• Build rapport with members to encourage positive behavior changes.
• Coach members to implement strategies to reduce health risks.
• Link members to local, county and state services/resources.
