Job summary

P3 Health Partners is seeking a Provider Engagement Manager to build and maintain relationships with primary care clinics and providers within a hybrid work model. The PEM will coordinate with Operations, Care Management, Quality, Risk Adjustment and Compliance to meet company goals and drive improved patient outcomes.

Expect 80-90% travel within the region, with in-office days as part of the hybrid schedule.

Qualifications

• 7+ years of experience in the health care industry working with an IPA/Medical Group, Health Plan, or hospital, preferably with direct physician interaction.
• 5+ years of combined experience in Medicare Advantage, CMS and/or CMMI programs such as Accountable Care Organization Model or DCE models.
• 5+ years of cross collaboration experience in complex project management and implementation.

Responsibilities

• Establishes new relationships with primary care clinic leadership, providers and staff, and maintains strategic relationships.
• Collaborates with Operations, Care Management, Quality, Risk Adjustment & Clinical Documentation and Compliance teams to achieve goals.
• Develops strategy, executes tactics, and drives performance for assigned clinics.
• Creates reports and dashboards to update leadership and track progress toward goals.
