EPIC Health is seeking a Care Management District Manager to oversee operational performance across clinics and districts, ensuring delivery of CMS-approved care management programs and care quality. This role partners with medical and administrative leadership to drive value-based care, improved outcomes, and strong financial results.

The District Manager will lead program implementation for CCM, PCM, APCM, TCM, BHI, RTM, and related CMS initiatives, while coaching managers and coordinating

Qualificações

• Bachelor's degree in Healthcare Administration, Nursing, Business Administration, Public Health, or related field.
• 5+ years of progressive healthcare operations leadership experience.
• Experience implementing and managing CMS care management programs.
• Experience in ambulatory care, value-based care, population health, or primary care operations.
• Strong knowledge of CMS reimbursement methodologies, quality reporting, and regulatory requirements.

Responsabilidades

• Lead operational leadership to execute the strategic vision for care management.
• Oversee implementation and adoption of new care management programs and workflows.
• Coordinate with clinic leadership and operational teams on implementation.
• Monitor progress, resolve barriers, and drive continuous improvement.
• Ensure compliance with workflows, SOPs, and organizational policies.
• Monitor KPIs and dashboards; develop corrective action plans when needed.
• Foster provider engagement and cross-functional collaboration.
• Support QA/QC initiatives and regulatory compliance.
