Centers Health Care is seeking a Centralized Admissions Specialist to focus on intake coordination and case management. This full-time role is based in Bronx, NY, working with admissions leadership to streamline processes and improve efficiency.

You will collaborate with hospital staff and providers to ensure timely referrals, accurate documentation, and compliant reimbursement practices while building strong relationships with community partners.

Responsibilities

• Work with Admissions management to improve the admission process.
• Collaborate with facility administrators and nursing directors to support admission needs.
• Coordinate resident referral and approval processes efficiently.
• Maintain relationships with local hospitals, physicians and community orgs to generate admissions.
• Understand admissions process including policy, procedures, assessments and paperwork.
• Maintain knowledge of regulations governing Medicare/Medicaid/Managed Care reimbursements.
