Job summary

August Healthcare is seeking an organized, responsive Centralized Admissions Coordinator to oversee referrals and admissions for our skilled nursing and post-acute care centers. You will connect prospective residents and families with care, coordinating with hospital case managers and center staff to ensure safe, timely admissions.

The role requires strong communication, multi‑tasking, and a collaborative approach, with evening/weekend/on‑call support as needed.

Qualifications

• High school diploma or equivalent required.
• Associate or bachelor’s degree in healthcare administration, business, social services, nursing, marketing, or related field preferred.
• Experience in skilled nursing, long‑term care, healthcare admissions, hospital discharge planning, insurance verification, case management, or related settings preferred.
• Familiarity with Medicare/Medicaid, managed care, private pay, and other payer sources preferred.
• Experience using referral management systems, hospital portals, and healthcare admissions platforms preferred.

Responsibilities

• Serve as central point of contact throughout the referral and admissions process.
• Collaborate with hospital case managers, discharge planners, referral sources, residents, families, and center teams.
• Review referrals for completeness and identify admission barriers.
• Coordinate admission dates, transportation, room availability, and related details.
• Provide timely admission notifications and maintain professional communication with stakeholders.
• Support census and occupancy goals by reducing delays in admissions.
