Job summary

Arc GLOW provides daily support to individuals with developmental disabilities in home settings. The DSP coordinates medical appointments and ensures communication and documentation to connect residents with proper care, while also delivering direct support as needed.

Responsibilities include assisting with medical services, maintaining medication supplies, coordinating with staff, and documenting activities. Training is provided, and a supportive team environment is emphasized.

Qualifications

• High School Diploma or equivalent required.
• Valid NYS driver's license with a safe driving record.
• Excellent communication skills.
• Willingness to learn new skills (paid training).
• Possesses computer skills.

Responsibilities

• Assist people supported in obtaining needed medical services and communicate/document per policy (e.g., medication administration, transportation to appointments).
• Ensure adequate medication supplies on site for each individual.
• Communicate with residential site staff to ensure orders are understood and followed.
• Provide recommendations to health care providers to maintain health and wellness.
• Complete required paperwork daily or as needed.
• Provide direct support as needed.
• Provide transportation using agency vehicles.
• Use various software platforms for job functions.

Job description

Our Care Providers provide daily support to individuals with developmental disabilities within the home setting. Working with a team, Medical Appointment Coordinators (MAC’s) are responsible for the coordination of all medical appointments and all necessary communication and documentation as needed to link individuals who live in residential sites to proper medical care. Responsible for providing direct support as needed.

WHAT WOULD I BE DOING?

• Responsible for assisting people supported in receiving needed/desired medical services and communicating and documenting pertinent information as per program policy (i.e., medication administration, transportation to appointments).
• Ensures an adequate amount of medication is on site for each individual at all times.
• Provides communication to residential site staff to ensure physician and clinical orders are understood and followed.
• Makes recommendation to the CSRN and/or Health Care provider to maintain and/or improve the overall health and wellness of individuals supported.
• Completes required paperwork on daily / as needed basis.

• Provides direct support as needed.

• Provide transportation as needed with agency vehicles.
• Use various software platforms for specific job functions.

QUALIFICATIONS FOR A DSP:
