Job summary

Foundations Health Solutions is seeking a State Tested Nursing Assistant (STNA) to deliver resident-centered care in a long-term care setting. You will assist with daily living activities, support mobility, and help preserve residents' dignity while collaborating with licensed staff to provide safe, high-quality care.

You will monitor health status, document vital signs, assist with meals, follow infection prevention protocols, and communicate effectively with residents and families.

Qualifications

Responsibilities

• Assist with activities of daily living (bathing, dressing, grooming, toileting, feeding, oral hygiene, mobility).
• Safely transfers and repositions residents using proper body mechanics and assistive equipment.
• Monitor residents' physical, mental and emotional well-being; observe and report changes to licensed staff.
• Assist with meal service and support residents with special dietary needs per care plans.
• Maintain clean, safe environment and follow infection prevention protocols.
• Collaborate with nurses, therapists, physicians and other staff to deliver individualized care.
• Provide emotional support, communication with residents and families.
• Adhere to facility policies, state regulations and professional standards.

Job description

The State Tested Nursing Assistant (STNA) provides compassionate, resident-centered care to individuals in a long-term care setting, assisting with activities of daily living such as bathing, dressing, grooming, toileting, feeding, oral hygiene, and mobility while promoting independence and preserving each resident's dignity. The STNA safely transfers and repositions residents using proper body mechanics and assistive equipment to ensure comfort and prevent injury.

The STNA monitors residents' physical, mental, and emotional well-being by observing and reporting changes in condition, measuring and documenting vital signs, and communicating concerns to licensed nursing staff. Responsibilities include assisting with meal service, monitoring food and fluid intake, and supporting residents with special dietary needs in accordance with individualized care plans.

The STNA maintains a clean, safe, and organized environment by following infection prevention and control procedures, practicing proper hand hygiene, and ensuring compliance with health and safety standards. Working collaboratively with registered nurses, licensed practical nurses, therapists, physicians, and other members of the healthcare team, the STNA helps deliver individualized care that supports each resident's overall health and well-being.

In addition to providing direct physical care, the STNA fosters positive relationships with residents and their families by offering emotional support, companionship, and encouragement. Through effective communication, teamwork, time management, and attention to detail, the STNA is committed to enhancing residents' quality of life while maintaining confidentiality and adhering to all facility policies, state regulations, and professional standards.
