Job summary

Kintegra Health in Gastonia, NC seeks a Clinical Pharmacist to provide oversight and advisement on pharmaceutical treatment for Senior TLC participants, reporting to the Senior TLC Medical Director.

Requires NC license, a PharmD or B.S. in Pharmacy, and at least three years clinical experience with geriatrics; responsibilities include therapy management, collaboration with physicians and nurses, and committee participation.

Qualifications

Responsibilities

• Provide clinical therapy management for Senior TLC participants.

• Review and manage medications with Senior TLC Providers during assessments and ongoing care.
• Facilitate pharmacy and care teams in polypharmacy meetings and committees.
• Monitor drug utilization and pricing patterns for quality and cost effectiveness.
• Lead Part D Risk Assessment processes with Quality and Compliance leadership.
• Participate in participant care planning and document care plans per protocols.

Clinical Pharmacist Summary:

This position collaborates with Senior TLC care providers and the Kintegra Pharmacy staff providing oversight and advisement regarding the pharmaceutical treatment for Senior TLC participants. The position reports directly to the Senior TLC Medical Director.

Education:

Bachelor of Science in Pharmacy or Pharm. D. degree.

Experience:

Three years of clinical pharmacy experience with one year of experience with a frail or elderly population.

Licensure, Registry or Certification Required:

Unrestricted Pharmacy license in the state of North Carolina

Special Training:

Ability to learn, teach and research; ability to solve problems and think analytically; must possess skills necessary to treat geriatric patients, ability to get along with others in a team environment. Must possess knowledge and skills necessary to treat patients; other training as required by department. Only act within the scope of his or her authority to practice. Meet a standardized set of PACE specific organization competencies for the specific position description established by Senior TLC prior to working independently. Embraces and supports the team-based PACE Model.

Familiarity with pharmacy software programs necessary to execute daily tasks & prior FQHC or public health experience preferred.

Clinical Pharmacist Key Responsibilities:

• Clinical therapy management
• Collaborates with physicians, nurses, and other healthcare professionals, providing education, communication and support for healthcare staff, participants, and families.
• Collaborates with Senior TLC Providers in medication review and management during assessments and ongoing as required.
• Facilitates monthly Polypharmacy meetings with Providers.
• Participates in the Senior TLC Falls Committee.
• Co-facilitates Pharmacy Committee with Kintegra Lead Pharmacist.
• Monitors drug utilization patterns for potential fraud and abuse TGH data analysis (including those prescriptions that are e-prescribed).
• Conducts data analysis of pricing data to detect outliers and inappropriate changes in formularies.
• In collaboration with the Senior TLC Director of Quality and Corporate Compliance, leads the Part D Risk Assessment process.
• Participates in participant care planning including the implementation of SMART goals/interventions for the participant's plan of care and enters all care plan information in a timely manner as per organizational protocols.
• Supports the Interdisciplinary Team and promotes unity among the team while interacting with the team, other co-workers, and/or participants.
• Supports team members and functions as a team player while providing that support and adheres to the "do no harm" team contract statement. Emphasizes teamwork and collaboration in all interactions.
• Supports Senior TLC's mission to encourage and support the quality of life of seniors wishing to continue living in the community; its vision to be the preferred provider of individualized care for seniors in the community; and its values of respect, integrity, accountability, compatible goals, and compassionate care.
• Other duties as assigned
