Position Summary

The Clinical Pharmacist is responsible and accountable for the provision of safe, effective, and prompt medication therapy. In this position, the Clinical Pharmacist provides direct patient-centered care and integrated pharmacy operational services in a medical home practice setting with physicians, nurses, and other medical personnel. The clinician in this role is aligned with target interdisciplinary programs to deliver medication therapy management within patient care services and to ensure pharmaceutical care programs are appropriately integrated throughout Central Florida Health Care clinics. The Clinical Pharmacist participates in all necessary aspects of the medication-use system while providing comprehensive and individualized pharmaceutical care to the patients in their assigned areas. Pharmaceutical care services include but are not limited to assessing patient needs, incorporating age and disease specific characteristics into drug therapy and patient education, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events. The Clinical Pharmacist serves as a departmental resource and liaison to other departments and will provide medication and practice‑related education and training to health care providers. Responsibilities may include working to establish and maintain long‑ and short‑term goals for the program; monitoring and documenting progress in meeting goals; and providing guidance to staff on priorities and clinical projects.

Minimal Qualifications
  • Graduate of an accredited College of Pharmacy
  • Doctor of Pharmacy or other equivalent advanced degree required
  • Completion of an accredited PGY1 Pharmacy Residency program required
  • Completion of an accredited PGY2 Pharmacy Residency program preferred
  • Clinical pharmacy experience (1+ years)
  • Knowledge of contemporary clinical practice and service
  • Knowledge and expertise in advanced pharmacy practice, medication therapy management, and evidence‑based medicine in areas appropriate for clinical assignments
  • Active Pharmacist License in good standing required
Responsibilities and Performance Expectations
  • Supports the Mission, Vision, and Values of CFHC
  • Be cognizant of and comply with CFHC’s policies and procedures, state, and federal regulations and URAC accreditation requirements
  • Maintains client/patient confidentiality by following and abiding by all HIPAA requirements and regulations
    • Leads assigned patient service lines, clinical areas, and therapeutic programs
    • Facilitate pharmaceutical care services, direct patient care programs, medication utilization systems within assigned services and care areas to assure drug utilization activities are aligned with patient care needs, evidence-based best practices, compliance, and regulatory standards
    • Assist in the implementation of antibiotic stewardship activities and restriction/surveillance programs
    • Participation in the Pharmacy and Therapeutic Committee which may include but is not limited to, Antibiotic Stewardship and Continuous Quality Improvement
    • Participate in quality management committees (URAC, QIRCS)
    • Leverage the 340B program to maximize the best clinical outcomes for our patients
    • Track and evaluate assigned pharmacy programs for operational, quality, and financial efficiency and routinely benchmark against local and national best practices
    • Provide MTM services to patients through CFHC preferred platform as available
    • Review documentation of clinical activities of all pharmacists including documentation of financial impact of clinical activities
    • Establish and ensure business metrics and key performance indicators are being met
    • Actively identify practice related issues which require evaluation and facilitate clinical research projects, quality improvement initiatives, or healthcare provider education as needed to advance practice
    • Implement and maintain policies, procedures, and operations to have compliant and successful clinical programs
    • Provide guidance on clinical related matters and ensure all levels of leadership understand impact and logistics of clinical business decisions
    • Develop advance reporting mechanisms for clinical pharmacy claims data representative of CFHC prescribing data and reports results to applicable stakeholders and committees
    • Generate and maintain required documentation for all clinical program audits and business processes
    • Collaborate with senior leaders and stakeholders to promote awareness, education and understanding of all clinical program principles for strategic and ongoing business decisions
    • Serve as a liaison between operations, providers, drug manufacturers, and PBMs
    • Coordinate clinical requirements related to manufacturer programs
    • Work externally with manufacturers and internally with providers, operations, and IT to ensure clinical program implementation and ongoing success
    • Work with the Clinical Pharmacy Manager and Chief Pharmacy Officer in identifying opportunities, developing strategic plans, and overseeing implementation of clinical pharmacy projects
  • Delivers direct patient care and clinical practice
    • Assure pharmacy is an integral part of the health‑care delivery system and facilitate enhancement and expansion of pharmacy services/programs
  • Complete critical patient monitoring and review patient charts to identify, prevent, or mitigate drug‑related problems, improper drug or dose selection, sub‑therapeutic dosage, overdose, adverse drug reactions, drug interactions, failure to receive drugs, untreated indications, medication use without an indication, and treatment failures
  • Identify high‑risk patients and implement interventions to improve adherence, outcomes and safety
  • Provide accurate, safe, timely, and appropriate medication therapy based on patient age and needs
  • Work as an active member of multidisciplinary team and collaborate with healthcare providers to provide patient‑centered care making appropriate evidence‑based, patient‑centered medication recommendations
  • Communicate effectively and appropriately with healthcare providers and caregivers (physicians, nurses, etc.) and ensure continuity of pharmaceutical care
  • Participates in pharmacy operations and medication dispensing
  • Maintain proficiency in and actively engage in operational programs and clinical pharmacy areas as appropriate for job assignment
  • Maintain proficiency in all applicable computer programs, including electronic medical record and pharmacy systems
  • Review, prepare, dispense, compound, and document pharmaceutical orders and products from CFHC and referral providers in line with state and federal regulations
  • Facilitates experiential education and practice advancement
  • Maintain proficiency in preceptor roles and actively emulates education/mentorship skills
  • Develop student, resident, and staff training experiences/competences and creates new relationships for teaching and training opportunities
  • Identify, design, and implement improvements in the medication‑use system to advance patient safety, maximize therapeutic outcomes, or control costs
