Job summary

Kintegra Health is seeking an experienced clinical pharmacist to oversee pharmaceutical treatment for Senior TLC participants in Dallas, NC. The role reports to the Senior TLC Medical Director and requires coordination with Pharmacy staff to ensure optimal care.

Ideal candidates bring 3+ years of clinical pharmacy experience, including geriatrics, and hold an unrestricted North Carolina license. Knowledge of FQHC/public health settings is a plus and a strong team player is essential.

Qualifications

• Doctoral-level pharmacy degree (Bachelor of Science in Pharmacy or Pharm.D.) and requires licensure.
• Three years of clinical pharmacy experience with at least one year with a frail or elderly population.
• Unrestricted North Carolina pharmacy license.

Responsibilities

• Clinical therapy management and collaboration with physicians, nurses, and other healthcare professionals.
• Medication review and management with Senior TLC Providers during assessments.
• Facilitation of monthly Polypharmacy meetings with Providers.
• Participation in the Senior TLC Falls Committee and Pharmacy Committee with lead pharmacist.
• Monitor drug utilization patterns for fraud and abuse and analyze pricing data for formulary changes.
• Lead Part D Risk Assessment process in collaboration with the Senior TLC Director of Quality and Corporate Compliance.
• Support care planning and enter care plan information per organizational protocols.
• Promote teamwork and support the Interdisciplinary Team to ensure quality of life for seniors.

This position collaborates with Senior TLC care providers and the Kintegra Pharmacy staff providing oversight and advisement regarding the pharmaceutical treatment for Senior TLC participants. The position reports directly to the Senior TLC Medical Director.

Education:

Bachelor of Science in Pharmacy or Pharm. D. degree.

Experience:

Three years of clinical pharmacy experience with one year of experience with a frail or elderly population.

Licensure, Registry or Certification Required:

Unrestricted Pharmacy license in the state of North Carolina

Special Training:

Ability to learn, teach and research; ability to solve problems and think analytically; must possess skills necessary to treat geriatric patients, ability to get along with others in a team environment. Must possess knowledge and skills necessary to treat patients; other training as required by department. Only act within the scope of his or her authority to practice. Meet a standardized set of PACE specific organization competencies for the specific position description established by Senior TLC prior to working independently. Embraces and supports the team-based PACE Model.

Familiarity with pharmacy software programs necessary to execute daily tasks & prior FQHC or public health experience preferred.

• Clinical therapy management
• Collaborates with physicians, nurses, and other healthcare professionals, providing education, communication and support for healthcare staff, participants, and families.
• Collaborates with Senior TLC Providers in medication review and management during assessments and ongoing as required.
• Facilitates monthly Polypharmacy meetings with Providers.
• Participates in the Senior TLC Falls Committee.
• Co-facilitates Pharmacy Committee with Kintegra Lead Pharmacist.
• Monitors drug utilization patterns for potential fraud and abuse TGH data analysis (including those prescriptions that are e-prescribed).
• Conducts data analysis of pricing data to detect outliers and inappropriate changes in formularies.
• In collaboration with the Senior TLC Director of Quality and Corporate Compliance, leads the Part D Risk Assessment process.
• Participates in participant care planning including the implementation of SMART goals/interventions for the participant’s plan of care and enters all care plan information in a timely manner as per organizational protocols.
• Supports the Interdisciplinary Team and promotes unity among the team while interacting with the team, other co-workers, and/or participants.
• Supports team members and functions as a team player while providing that support and adheres to the “do no harm” team contract statement. Emphasizes teamwork and collaboration in all interactions.
