Job summary

Commonwealth of Massachusetts seeks a Transition Program Coordinator in Boston to support the Chapter 688 Turning 22 process and transition planning for youth aging out of school services. You will manage data, generate reports, and coordinate timelines across DDS and DESE, engaging families and schools for seamless implementation.

You will provide training on CH 688 requirements, IEPs, and HUB functionality, while assisting with budget development and program improvements.

Qualifications

• Experience with transition planning and IEPs is preferred.
• Ability to analyze data, prepare reports, and forecast needs.
• Familiarity with state education and transition policies is desirable.

Responsibilities

• Coordinate statewide chapter 688 Turning 22 processes and timelines.
• Analyze data in DDS HUB and Virtual Gateway to support planning and budgeting.
• Develop and distribute Turning 22 reports to regional offices.
• Provide training and guidance on transition requirements and program tools.
• Collaborate with DDS staff, schools, families, and partners to meet program goals.
