Hartford HealthCare at Home is seeking a dynamic Supervisor of Social Work to lead certified social work services across the state. You will supervise staff, partner with regional leaders, oversee care planning, documentation, and regulatory compliance, and ensure high-quality patient care.

Travel is required to statewide service areas; you will mentor teams, drive performance, and collaborate with interdisciplinary colleagues to optimize outcomes and staffing across Hartford HealthCare's home

Qualificações

Must hold a Master of Social Work from a CSWE-accredited program.
Active Connecticut LCSW license required.
Valid CT Driver’s License.
3–5 years supervisory experience in Home Health.
3–5 years clinical social work experience in healthcare.
3–5 years’ experience using an EMR system.

Responsabilidades

Lead and supervise Social Work staff across the state; ensure quality patient care and regulatory compliance.
Schedule staff, manage performance, and provide training and onboarding for new employees.
Collaborate with region leaders on day-to-day operations and budgets; monitor KPIs and prepare reports.
Perform field social work visits (3–5 per week) to support statewide teams and care planning.
Contribute to discharge planning, total care plans, and interdisciplinary case conferences.
