Job summary

Qualifications

Responsibilities

• Performs comprehensive in-home psychosocial assessments.
• Collaborates with interdisciplinary team to develop and implement care plans.
• Provides short-term interventions, crisis intervention, and counseling as needed.
• Educates patients, families, and caregivers about disease, treatment options, and community resources.
• Advocates for patient needs and coordinates discharge planning and transitions of care.
