Job summary

Home Health Care, Inc. is seeking an Admissions RN to lead Start of Care for the Twin Cities Metro, ensuring each patient begins care with confidence and a clear plan.

You will perform comprehensive assessments, coordinate with physicians and care teams, educate patients and families, and develop individualized plans to set patients up for long-term success in home health. This role offers territory-based scheduling, autonomy, and the chance to impact patient outcomes from day one.

Qualifications

• Active Minnesota RN license.
• Home health and OASIS experience strongly preferred.
• Excellent communication and patient education abilities.
• Ability to work independently in a field-based role.

Responsibilities

• Complete SOC admissions throughout the Twin Cities Metro.
• Perform detailed nursing assessments and complete accurate OASIS documentation.
• Develop individualized plans of care that set patients up for long-term success.
• Educate patients and families on diagnoses, medications, services, and expectations.
• Coordinate seamlessly with physicians, case managers, therapists, and the interdisciplinary care team.
• Ensure smooth, safe transitions from hospitals, SNFs, and clinics into home health.
