Job summary

Home Health Care, Inc. seeks an RN Admissions Nurse to lead Start of Care visits for patients transitioning into home health. You will perform comprehensive assessments, document OASIS, and craft individualized care plans for long‑term success.

You will educate patients and families, coordinate with the care team, and ensure safe transitions from hospital or facility to home health. This role requires RN licensure in Minnesota and strong communication skills.

Qualifications

• Active Minnesota RN license.
• Strong clinical assessment and critical thinking skills.
• Home health and OASIS experience preferred.
• Exceptional communication and patient education abilities.

Responsibilities

• Complete comprehensive Start of Care admissions throughout the Twin Cities Metro.
• Perform detailed nursing assessments and complete accurate OASIS documentation.
• Develop individualized plans of care for long‑term success.
• Educate patients and families on diagnoses, medications, services, and expectations.
• Coordinate with physicians, case managers, therapists, and the care team.
• Ensure smooth transitions from hospitals and facilities into home health.
