Job summary

Interim HealthCare Inc. in Minnesota seeks an Admissions Nurse (Start of Care SOC Visit RN) to provide direct patient care in patients' homes. You will assess needs, develop and follow the plan of care, and collaborate with an interdisciplinary team.

This full-time daytime role covers the Twin Cities Metro area with travel throughout Roseville, MN. Licensed RN in Minnesota with CPR is required; two years of RN experience and home care/hospice experience preferred.

Qualifications

• Licensed RN in Minnesota in good standing.
• Must be able to travel within the office geographic area and drive.
• Two years of RN experience required.
• One year home care or hospice experience preferred.
• CPR certification required.

Responsibilities

• Completes admissions or visits for the Home Care team.
• Direct patient care under supervision of Nursing leadership.
• Create and follow the plan of care.
• Provide and determine treatments for patients.
