Job summary

RHA Health Services, LLC in North Carolina seeks a START Coordinator to support individuals with developmental disabilities and mental health needs in the community. You''ll collaborate with a multidisciplinary team to develop crisis plans and connect families with services.

This full-time role requires a Bachelor’s degree in Social Work or related field, 2+ years of experience, valid driver''s license, and willingness to travel; CPR/First Aid and NCI certifications preferred.

Qualifications

• Bachelor’s degree in Social Work, Social Services, Human Services or Special Education.
• Master’s degree a plus.
• Minimum 2 years experience with intellectual/developmental disabilities; 1 year if paired with Master’s.
• Must meet QDDP and QMHP requirements with direct support and crisis experience.
• Valid driver’s license and ability to travel.
• CPR/First Aid/NCI certification or equivalent.

Responsibilities

• Develops and revises cross systems crisis plans in a timely fashion.
• Monitors the implementation of the plan.
• Performs Case Management functions linking and arranging services and referrals.
• Provides families and stakeholders with training on required knowledge and skills.
• Provides consultation to the system on the clinical needs of the individual's care.
• Ability to travel as required.
